Disability Access and Inclusion Plan

We are committed to creating an environment that enables people with disability, their families and carers, to have full access to all North Metropolitan Health Service (NMHS) services, facilities and information.

The NMHS Disability Access and Inclusion Plan (DAIP) for 2017–2022 (PDF) was devised in accordance with the statutory responsibilities of the Disability Services Regulations 2004 and other related legislation such as the Equal Opportunity Act of 1984. This plan acknowledges the Department of Health’s 2016 – 2020 DAIP and the requirement for all staff to actively work towards progressing better access and inclusion in our workplaces.

Please email NMHS.SQGCE@health.wa.gov.au if you require a copy of the plan mailed to you.

The anonymous survey (external site) takes 5-10 minutes to complete and closes midnight Friday 15 July 2022.

The survey can be provided in different formats. If you need a different format or you have any questions or concerns about disability access and inclusion at NMHS, contact Lanny Hoskin, Safety and Quality Coordinator, on 6457 1736 or NMHS.SQGCE@health.wa.gov.au.
