Providing evidence* about your booster status is as easy as 1,2,3


  1. Obtain evidence of your vaccination

    Take a photo of your full vaccination certificate that is accepted by the Directions and save it in your phone library/onto your PC

    Accepted forms of evidence include:

    1. written confirmation of vaccination issued by Department of Health
    2. a COVID-19 Digital Vaccination Certificate or Immunisation History Statement showing COVID-19 vaccinations recorded on the Australian Immunisation Register. This can be accessed via Medicare and myGov.
    3. an International COVID-19 Vaccination Certificate issued by the Commonwealth Government

  4. Complete the form

    Answer 5 questions, and upload your vaccination certificate photo from your photo library/PC and submit.


*WA Health staff are now required to get a third (booster) COVID-19 vaccination before 5 February 2022 (or if yours is not due by this date, you will need to receive it within one month of becoming eligible to receive it). As part of the mandate, you will also need to provide evidence of your booster vaccination.
